He earned his MBBS from Government Medical College, Trivandrum, before pursuing postgraduate specialisation at JIPMER, Puducherry — an Institute of National Importance — where he was awarded the Gold Medal for his postgraduate examinations. He went on to complete the MRCS from the Royal College of Surgeons of Edinburgh, UK.

To refine his surgical expertise, Dr. Karunakaran completed elite fellowships across the globe — including training under Dr. David Rajan at Ortho One, Coimbatore, and advanced international fellowships in Shoulder & Knee Surgery at Sydney, Australia (Australian Orthopaedic Association) and in France with world-renowned surgeons.
